Behavioral Health Admitting Process

Our Community Assistance Program (CAPs) is TriStar Health’s primary point of entry. Team members are available 24/7 to assist health care providers, families and individuals with understanding mental health issues and making a level of care assessment.

Staffed by master’s-level mental health counselors and psychiatric registered nurses, CAPs provides immediate response to telephone inquiries, suggestions for appropriate action and information about community mental health resources. CAPs also offers:

  • Confidential assessments and consultations
  • Referrals to outpatient, intensive outpatient, partial hospitalization or inpatient care based on each individual’s treatment needs
  • Support for families of those with mental illness
  • Information about mental health concerns and community resources

Patients are assessed as follows:

  • A patient or referral source (PCP, psychiatrist, therapist, mobile crisis team, emergency room, family member/friend, etc.) contacts CAPs by calling (615) 342-1450 or (800) 727-2345.
  • CAPs gathers clinical and demographic data to determine whether an assessment is indicated.
  • If the individual does not require an assessment, CAPs will provide appropriate referrals to a wide continuum of mental health resources in the Middle Tennessee area.
  • If an assessment is needed, CAPs schedules it and arranges for the first available assessment specialist to see the patient upon arrival at TriStar Health or one of our satellite offices.
  • NOTE: Individuals may come directly to TriStar Health without calling to schedule an assessment. Prior evaluation at the ER is not required.

Requested Information

Because collaboration is critical for treatment success, CAPs staff will collect the following information:

  • Clinical information, such as risk of suicidality, homicidality, danger to others and the inability to care for self
  • Chronic and current medical conditions and an up-to-date medication list
  • Past treatment history and any special care needs
  • Insurance information (CAPs will ensure TriStar Health and the attending psychiatrist are approved providers and will obtain pre-authorization.)
  • Evaluation and documentation for involuntary patients (CAPs will complete and submit this paperwork.)

  • List of current medications and allergies
  • Insurance card
  • List of emergency phone numbers
  • 3 changes of comfortable clothes (no drawstrings or wire hangers)
  • 1 pair of pajamas (no drawstrings)
  • Sweater or Jacket
  • Flat, Non-Skid Shoes (no shoestrings)
  • Dentures, glasses and hearing aids
  • Picture of family or pet
  • Favorite books or magazines
  • List of favorite foods/food allergies
  • Please be sure to write patient’s name on belongings

Outpatient Behavioral Health Outreach Coordinator

People sometimes put off taking care of their emotional well-being because they don't even know where to start. Your mental health matters and we encourage you or your loved one to talk with a trusted behavioral health specialist when you need help.

Our Mental Health Coordinator arranges outpatient mental health services based off of your best interests.

Our coordinator works with patients, families and referring providers to assist with:

  • Appointment scheduling
  • Insurance verification
  • Coaching
  • Education
  • Mental health resources in the Nashville area

Confidentiality

To protect each patient’s right to privacy, hospital policy and federal patient privacy laws require family members and friends to know the patient’s room number in order to contact their loved ones while hospitalized. Patient’s who wish for family/friends/significant others to participate in their treatment and have access to protected information, may sign a release of information stating who they want involved in their care and the level of access.

Restrictions

  • Do not bring valuables
  • No luggage, duffel bags, suitcases, belts, or purses/wallets
  • No personal canes, wheelchairs or walkers (the hospital will provide the patient with these needs)
  • No cell phones
  • No photography
  • No children under the age of 16

From I-65: Take Exit 209/Charlotte Ave. Drive west on Charlotte Ave. away from downtown). Turn left on 24th Ave., which dead ends into the main entrance for TriStar Health at 2401 Parman Place.
