To register online at TriStar Centennial Medical Center, sign in using your MyHealthONE® account.
Online registration is only available for the following service types:
- Surgery, procedure or scheduled appointment
- Scheduled screening mammogram
- Childbirth
What you will need
Please make sure you have all necessary information available to complete this process. You will need:
- All contact information (patient, responsible party, and emergency contacts)
- All current insurance cards (including Medicare and Medicaid)
- Date of scheduled visit or expected delivery date
- Family and ordering physician first and last names
- Employer contact information
TriStar Centennial has six separate registration areas. They are located at TriStar Centennial Tower, TriStar Centennial Imaging Center, TriStar Centennial Women's & Children's, Women's Health Center, Emergency Department, TriStar Parthenon Pavilion, Physical Therapy Registration and Atrium Plastic Surgery Registration.
TriStar Centennial Tower
The hours of operation at the TriStar Centennial Tower Registration are Mon - Fri: 5:30am - 6:00pm. All patients entering the Tower for any inpatient stays or outpatient tests and procedures must first come through Registration at the Tower. The Registration Desk is located on the first floor of the Tower in the main lobby and may be reached at (615) 342-1547.
TriStar Centennial Imaging Center
The hours of operation are Mon - Fri: 8:00am-4:30pm. Located at 356 24th Avenue North (corner of 24th and Charlotte Avenue.
TriStar Centennial Women's & Children's
The hours of operation are 24 hours a day, 7 days a week. OB patients that present believing they are in labor should go directly to OB Assessment Center on the 6th floor for observation.
Patients entering TriStar Centennial Women's & Children's for tests and procedures must first come through Registration, Mon - Fri: 6:00am - 6:00pm, which may be reached at (615) 342-1575 and is located on the first floor of The Women's Hospital.
TriStar Centennial Women's Health Center
The hours of operation are Mon - Fri: 7:30am - 4:30pm. The Women's Health Center is located on the 3rd floor of the Women's Hospital in Suite 115. The Women's Health Center can be reached at (615) 342-5018.
TriStar Centennial Emergency Room
The hours of operation are 24 hours a day, 7 days a week. All emergency patients entering TriStar Centennial through the Emergency Room will be registered in this area. Any admissions occurring after TriStar Centennial Tower Registration hours will also register through this area. The Emergency Room may be reached at (615) 342-1500 and is located on the first floor of the TriStar Centennial Tower at the corner of Leslie Avenue and 23rd Avenue North.
TriStar Parthenon Pavilion
TriStar Parthenon Pavilion conducts a free assessment prior to admission. The hours of operation are 24 hours a day, 7 days a week.
